Note: You must print and complete Part 1 of the Medical Check-up Form (MC1 Form) and bring it on your appointment day. Refer to the MC1 Form Guide. You may complete your medical check-up at any non-RMG medical clinics/hospitals in overseas or Singapore. If your check-up is at a non-Raffles Medical clinic, show Part II of the Guide to the Physician completing your form.
